Previously, the patient tried a daily wear omafilcon B (62%) multifocal contact lens design, wearing a sphere in the right eye and toric in the left eye. However, after various trials he remained unsatisfied with both comfort and vision, particularly his near vision for work. The practitioner shared that the patient was only able to achieve 20/30 vision (with effort) at distance and near with his previous multifocal contact lenses. The following information was provided to design a set of trial custom soft multifocal contact lenses: **Keratometry Readings:** OD: 44.12@177/44.87@87 OS: 44.5@12/45.25@102 **HVID:** OD: 11.50mm OS: 11.50mm **Manifest Refraction:** OD: -3.0 -.50 x167 (+1.75 add) OS: -4.0 -1.0 x003 (+1.75 add) **Pupil Size:** 4.0mm in a regular room light setting Using the [SpecialEyes Arc Length Calculator](https://calculators.specialeyesqc.com/arc-length-calculator.php) and [Multifocal Simulator](https://calculators.specialeyesqc.com/multifocal-simulator.php), the following trial contact lenses were ordered and dispensed. **SpecialEyes 54 Multifocal Toric Trial Lenses:** OD: 8.00 / 14.4 -3.00 -0.50 x167 (+1.75 Add) OS: 8.00 / 14.4 -3.75 -1.00 x003 (+1.75 Add) OU Multifocal Optic Zones: 2.0mm near center zone and 3.5mm peripheral zone ## **The outcome** Upon insertion, the patient was thrilled with the comfort and visual performance of the SpecialEyes 54 Toric Multifocal contact lens design. During a visual acuity assessment the patient was able to see 20/20 (-) at distance and 20/15 at near. There was no over refraction and slit lamp assessment revealed a good fit with no lens rotation. The patient was instructed to wear the lenses for a week to ensure vision and comfort remained stable.
